Thanks so much for reporting these things. We've been talking with Microsoft a lot over the last 24 hours and will continue to do so. The freezing and the achievement error are not something we saw in the test environment they provide to us, and aren't something we have been able to reproduce here in either environment, so it appears to be a difference between how our game behaves in the test environment vs. how the same game behaves in the production environment for *some* people. Already Microsoft found one way in which they hadn't set up things as they were supposed to be in the production environment and we'll work with them to see if there are other ways as well.
Our goal is to help MS find any issues on setup, or get a repro case that we can see under the debugger and determine what is happening. We're on that and will do what we can.

Yep, a personal account recovery, after the data center has the info it should have had to start with, should fix the issue.
The strange screens come from a situation that we still don't completely understand where you are somehow able to get to the mission selection screen before all the content is loaded. Bad things will eventually happen when you get into that state. So if it happens to you, back out to the press start menu, and wait there five or six seconds longer than you would normally before proceeding to the main menu. We don't understand why you should have to do this (you shouldn't), but that gives the game longer to load all the content. You'll know things are fine when you see regular text on the menu selection screens and not $variable
What about the achievements not unlocking? I really can't play this game again until that is sorted out.
We were told by Microsoft that the achievements not unlocking was due to the achievements file not being deployed by Microsoft until a day after the game was out. If you recover your gamertag and then play the game, it should unlock all the achievements you've earned after it does the first check for new acheivements (say at the end of any level). This is now working for some people, and not for others, so we've contacted Microsoft to see if the acheivements file on the back-end still needs to be deployed further and for some reason isn't in all data centers.
